A. SCHEDULE OF AVAILABILITY OF SERVICE

  • 8:00 a.m. to 5:00 p.m. without noon break

B. WHO MAY AVAIL OF THE SERVICE

  • Person needing social welfare development assistance
  • Individuals/families in crisis situation 
  • Who have not yet availed of CIU assistance during the year. 
  • Who are indigent as certified by the Barangay Chairman of the area where they resides.

 C. REQUIREMENTS

  •  For Hospitalization/Medical Assistance
  1. Clinical Abstract and/or
  2. Medical Certificate with license/PTR No. of the attending physician
  3. Prescription (for medicine)
  4. Laboratory Request/Medical Procedures (recommended by the attending Physician)
  5. Billing Statement (for hospital bill) Social Case Study Report from LGU (optional) 
  6. Barangay Certification/Indigency/ any valid ID of the patient or his/her representative (e.g. driver’s license, postal, school, voters and senior citizen ID) 
  7. Referral/Endorsement letter from legislator, if applicable.
  • For Burial Assistance
  1. Registered Death Certificate
  2. Funeral Contract
  3. Permit to Transfer (transport of cadaver)
  4. Barangay Certificate/any valid ID
  5. Social Case Study report from LGU (optional)
  6. Referral/Endorsement Letter from legislator, if applicable
  • For Transportation Assistance
  1. Barangay Certification/Indigency/any valid ID
  2. Social Case Study Report from LGU (optional)
  3. Referral/Endorsement letter from legislator, if applicable
  • Educational Assistance
  1. Certificate of Enrollment
  2. Barangay Certification/Indigency/any valid ID
  3. Referral/Endorsement letter from legislator, if applicable
  4. Social Case Study Report (optional)

D. PROCESSING TIME

  • For cash outright  –  Maximum of 45 minutes per client 
  • For issuance of guarantee/referral letter – Maximum of 15-20 minutes per client
  • For processing of check – Three (3) to five (5) days

E. HOW TO AVAIL OF THE SERVICE

  3. Recommendations/ Social CIU Office Actions Taken for the Worker Sta. Barbara assistance requested Zamboanga City or at Prov’l. SWADTs.
  4. Review and Approval Officer-In- CIU Office of the Officer-in-Charge Charge or Sta. Barbara to the Social Worker’s SWADT Leader Zamboanga City recommendation for the or at assistance being requested Prov’l. SWADTs
  5. Client will receive the approved assistance:
    • Financial- Cash OIC of CIU or CIU Office Outright SWADT Leader/ Sta. Barbara Designated Zamboanga City Disbursing Officer or at Pro’vl. SWADTs
    • Financial- OIC of CIU or CIU Office Guarantee/Referral SWADT Leader Sta. Barbara Letter Zamboanga City or at
      Prov’l. SWADTs
    • Financial- Check Regional Cashier Cash Unit Or thru DSWD FO IX SWADT Leader Zamboanga City or at Prov’l. SWADTs

6. Client to affix their OIC of CIU CIU Office signature as proof for SWADT Leader SWADT Offices the assistance given Disbursing Officer DSWD FO IX Cashier Zamboanga City
